VAAFT or Video Assisted Anal Fistula Treatment is a newer technology applied for the surgical treatment. An anal fistula is understood to be an abnormal connection found in between the anal outer skin and anal canal. Fistulas are usually occurred when the infected anal glands do burst to internal anal canal. The bursted ones will be conduit for discharge of pus to the area of the anal skin. The pus discharge gives the wet feeling at the area of anal to the patient.

Before VAAFT Procedure

Biochemical investigation is performed before the surgery takes place and patients with diabetes will be ruled out. Patients, who have crohn’s disease or tuberculosis are also ruled out.

The technique of VAAFT is divided into two stages, called diagnostic stage and operative stage.

Diagnostic Stage

During the diagnostic stage, the right and accurate location of the fistula tract is identified. the fistuloscope is used to insert through the opening made externally. The scope is then advanced and the lumen is visualized directly.

Operative stage

The patient is initially given the anesthesia. Then, a telescope of smaller size is inserted through the fistula-in-ano outer opening. The tube is connected to the HD camera, so the inner architecture or structure can be viewed on the monitor, as it transmits the images on the HD monitor. The total length of the fistula tract is identified by viewing on the monitor and then it is removed completely, till the internal opening that is seen right from inside the wall of it and anal canal. At the same time, fistula tract branch are also identified, if these are present and then are also treated. After destruction of the fistula, the canal of fistula is cleaned, by removing the waste material. After the treatment, the internal opening will be covered with the help of the stapler or stitches. It is done to not leak the feces or stool, back to the tunnel of fistula.

Recovery

Post surgery, the patients will be able to take orally after six to eight hours the procedure is performed. After the surgery is completed, the patient needs daily dressing that is done by flushing with the help of saline and also using a syringe for removing the issues that are dead from inside fistula tract healing. Patient has to make the revisits to the hospital after 15 days of the operation, followed by one month and three months.
