Lumbar corpectomy, or anterior lumbar corpectomy is a surgical procedure, which is performed for spinal decompression. The damaged tissues and vertebrae that cause the pinch of the nerve roots are removed with the help of this procedure. The spine will then be stabilized with the help of a cage or graft by inserting in the spine.
Apart from this objective, anterior lumbar corpectomy is performed also for the correction of spinal deformities so that the pain caused from the damaged or diseased vertebrae could be relieved. The pain is caused from the vertebrae that pinch the spinal nerve roots. The surgery is performed, after attempting the regular and conservative treatment method and failing from the same. It also is used for the decompression of the spinal cord and/or spinal nerves that are pinched.
Need for the Procedure
Lumbar Corpectomy is performed for the following symptoms and spinal problems.
- Spinal tumor
- Fractured vertebrae
- Spinal infection
- Diseases like degenerative disc
Symptoms
- Weakness, numbness or stiffness experienced at the lower extremities
- Mild chronic pain to severe chronic pain felt at low back and also sharp, burning, throbbing, dull or shooting
- Pain that spreads or radiates to the chest and ribs from the middle back
- Pain radiating into the legs, toes, feet, calves from the lower back
- Unable to perform daily and regular activities
- Difficult to sit, walk or stand for long period of time
- Decreased motor skill, felt at the lower extremities
- No help even after attempting conservative treatment methods
Procedure
The procedure is started after giving general anesthesia. The entire procedure is completed from 1 to 3 hours of time and the exact time, depends on the complexity. Initially, the left side of the patient is made ready for the incision and surgery. The bone graft can be considered to be taken in one of the two ways. The bone graft can either be taken from the patient’s own or can be preferred to take from the bone bank. If the patient has preference of the own bone, then the initial incision is done at the hip. Alternatively, a titanium cage can also be filled with the help of bone graft to bridge the vertebrae space.
The surgery continues with removal of the vertebral bone part that has been diseased for giving relaxation to the nerve. The vertebral discs present below and above of the vertebra would also be taken out or eliminated. Additionally, if there are any of the spinal abnormalities, these are also eliminated. The surface of the bone is cleared and it is made ready to be used as a bone graft. Bone graft extraction is done, if applicable. Two bolts are taken to screw into the below and above of the vertebrae. After screwing, certain tools specific to that purpose are hooked onto the bolts. It makes the vertebral space to be opened. The overall curvature of the spine would then be corrected.
Then cage or bone graft is then used to fill the space in the vertebrae. The fusion is done by taking a small metal plate and attaching it to two bolts, with the help of two more screws. After the operation is performed, incision would then be closed using the sutures.
